РЕЗЮМЕ
В обзоре литературы представлены последние данные по клиническим исследованиям в гомеопатии. Обсуждаются возможные условия проведения исследований по общим правилам и необходимость дискуссии по вопросам наиболее адекватного изучения клинической эффективности гомеопатических препаратов. Высказывается мнение о необходимости разного подхода к исследованиям в зависимости от предназначения гомеопатического препарата – применения его по медицинским показаниям, либо индивидуализировано, либо по правилам гомеопатии в соответствии с аффинитетом препарата.
Ключевые слова: гомеопатические лекарства, клинические исследования, плацебо-контролируемые слепые исследования, дизайн клинического исследования для гомеопатического лекарства.

RESUME
This article provides an overview of modern clinical investigations in homeopathy. Various conditions of these investigations are discussed. A need of metodological discussion to look for optimal way for determining of homeopathic remedies efficacy is discussed. There is a signed opinion that there must be various investigation ways depending on kind of using of homeopathic remedies – classical way, way of symptomocomplex and affinity of remedy, or in accordance with medical indications.

Введение

Борьба за рынок лекарственных препаратов ведется с тех самых пор, как стала развиваться фармацевтическая промышленность. Настоящий период времени также имеет все признаки этой борьбы, которая характеризуется конкурентными взаимоотношениями между разными производителями фармпродукции и их совместным противодействием развитию производства гомеопатических лекарств и увеличению их потребления. Именно это лежит в основе периодически появляющихся всплесков информации о том, что кто-то уже запретил или надо запретить применение гомеопатических лекарств. Например, в Меморандуме Королевского фармацевтического общества Великобритании 2010 г. [56] сказано о том, что необходимо развить методы, которые смогут доказать эффективность гомеопатических лекарств, а пока этого нет, следует отказаться от идеи об эффективности и информировать пациентов о том, что нет никаких научных оснований для существования гомеопатии. Кроме того, следует изучить вопрос о том, что именно привлекает пациентов – эффективность лекарств или самого «гомеопатического пакета услуг», в котором важную роль играют психологические аспекты консультации. Следует разобраться не компрометирует ли гомеопатия своим существованием и конвенциональную медицину, а также выяснить соотношение «цена-эффект» для гомеопатического лечения.

Частично в такой постановке вопроса есть вина самих гомеопатов, которые так уверены в действии лекарств, что пренебрегают принятыми методами оценки и не имеют или имеют недостаточно доказательного материала для опровержения.

Однако следует учитывать факт того, что гомеопатия, как и вся традиционная медицина, построена по другому принципу – тут в основе врачебного подхода лежит методология, характерная для каждого направления традиционной медицины, а не формализованная технология, принятая для оценки эффективности в доказательной медицине. Это и является тем самым «узким местом», «слабым звеном» малой (в смысле сегмента рынка), но значительной (в плане многовековых традиций) традиционной медицины, к которой все более тяготеет население разных стран. Для оценки малочисленных исследований, к каковым, как правило, относятся гомеопатические, часто ретроспективно применяют метаанализ для объединения имеющихся данных в большие статистически значимые группы. При этом неизбежны ошибки, которые несет в себе формализация непредназначенных для этого данных, относящихся к индивидуализированному лечению. Образно выражаясь «как бы не суммировать яблоки с апельсинами, а иногда и с лимонами». Kienle G.S. и соавт. [41] считают, что пора начинать методологический диалог. Не отрицая заслуг доказательной медицины (прозрачность в принятии клинического решения, высвобождение от гнета опиньон-лидеров, критический анализ лечебной рутины и многое другое), авторы говорят об ограничениях этой технологии и предлагают варианты выхода для решения вопроса. Например, они говорят о том, что имея в основе медицины врача, клинические испытания не учитывают его роли вообще, не учитывают особенностей пациента, делая испытания в клиниках, не учитывают, что на первичном приеме у врача общей практики большинство пациентов имеют еспецифические жалобы, часто не имеют диагноза, а иногда диагноз сочетается с другими тяжелыми заболеваниями, являющиеся критериями исключения при проведении рандомизированных клинических испытаний. Вопрос о том, как эффективное при одной патологии фармсредство влияет на развитие другой патологии остается по-прежнему открытым. Данные по исследованию потока заболевших среди населения, проживающего в США, свидетельствуют о том, что из тысячи людей только один попадает в условия, соответствующие проведению клинических испытаний (табл. 1).
