Publication 2: analysis of the results of clinical studies

РЕЗЮМЕ
Из массива библиографических источников различной степени достоверности нами выявлены и подвергнуты анализу результаты клинических исследований алкоксиглицеридов (AKG), очищенного масла печени акулы «Экомер» (Ecomer®) (Натумин Фарма, Швеция) и неочищенного масла печени акулы. Установлено наличие доказательной базы по применению AKG и стандартизированного продукта из масла печени глубоководных акул северных морей «Экомер» (Ecomer®) в качестве иммуномодулирующего и противоопухолевого средства в зарубежных клиниках. Результатов применения алкоксиглицеридов и стандартизированного продукта из масла печени глубоководных акул северных морей «Экомер» (Ecomer®) в отечественных клиниках нами не выявлено.
Эффективность, безопасность и спектр фармакотерапевтической активности продукции на основе масла печени акулы зависит от качественного состава, количественного содержания ингредиентов, а также степени и характера очистки исходного сырья. Поэтому нельзя считать правомерной экстраполяцию выявленной нами доказательной базы в части иммуномодулирующего, гипохолестеринемического, противоопухолевого действия и отсутствия токсичности AKG и стандартизированного продукта «Экомер» (Ecomer®) на более дешевую линейку БАД к пище из масла печени акулы различной степени очистки и качества.

RESUME
We have identified and analyzed the results of clinical studies of alkylglycerols (AKG), purified shark liver oil (Ecomer®, NatuminPharma, Sweden) and crude shark liver oil in bibliographic sources of varying degrees of reliability. Studies performed abroad (worldwide) recently confirmed the earlier studies by A. Brohult (for AKG) and clearly verified anticancer and immunomodulatory effect of Ecomer®. We have not identified the reliable results of applying of AKG and standardized product from the deep-sea shark liver oil of the northern seas Ecomer® in Russian clinics.
Extrapolation of the evidence base of the immunomodulating, antitumor effect and lack of toxicity AKG and standardized product Ecomer® to cheaper shark liver oil food supplements of varying purity and quality is incorrect.

Библиографические данные о доклинических исследованиях, касающиеся специфичес-кой фармакологической активности и безопасности (токсичности, мутагенности) продуктов из неочищенного (нефракционированного) [4, 6, 7, 11, 12, 17, 37, 59, 70, 75, 87] и очищенного жира печени акулы [5, 15, 51, 52, 71–73, 77], а также фракций алкоксиглицеридов (AKG) [1, 5, 16, 38–41, 49, 65, 68, 85] и сквалена [11, 53, 54, 75, 76, 87], чрезвычайно неоднородны. Ранее нам удалось обосновать недопустимость экстраполяции результатов токсикологических и фармакологических исследований стандартизированной продукции, содержащей очищенные фракции жира (масла) печени акулы, на разнообразную продукцию (БАД к пище) из неочищенного масла (различных биологических видов рода акула), не подвергавшуюся аналогичным испытаниям [3]. Актуальность проведенного информационно-аналитического исследования диктовалась наличием подобных попыток экстраполяции в популярных и научно-популярных статьях.

Достоверные результаты экспериментальных исследований позволяют считать доказанными иммуномодулирующее и противоопухолевое виды действия очищенных фракций AKG, а также стандартизированного продукта из масла печени акулы «Экомер» (Ecomer®) [3]. Эти данные согласуются с традиционным применением масла печени акулы в Швеции и Норвегии [9, 45, 48]. Известно также, что первоначально масло печени акулы добывали скандинавские моряки для лечения кожных и простудных болезней, рака и заболеваний желез внутренней секреции, которые в настоящее время классифицируются как лимфаденопатии [9, 45, 51, 76]. Очищенное масло печени акулы «Экомер» (Ecomer®) или отдельные его компоненты, например AKG, сегодня используются в онкологии (в том числе, в восстановительном лечении) [32–34, 42, 45, 48, 50–52, 56, 58, 61, 68, 69], а также в качестве иммуностимуляторов (иммуномодуляторов) [42, 45, 48, 49, 56, 61, 67–69]. Сообщается о достоверно установленном влиянии продукции на основе жира акулы на синтез и обмен ряда аминокислот в организме [49] и выраженном антиоксидантном действии [1, 49, 67, 69]. При этом зачастую результаты клинических исследований отдельных фракций или видов продукции на основе жира акулы безосновательно переносятся на исходное сырье или многочисленные БАД к пище разного состава и качества, для которых подобные исследования не проводились.

Актуальным является информационно-аналитическое исследование результатов клинических испытаний всего спектра продукции на основе масла печени акулы различного происхождения, состава и качества (фракций AKG, сквалена, неочищенного и очищенного масла), в сравнительном аспекте.

Целью настоящей работы является сбор, обобщение и анализ результатов клинических исследований спектра фармакотерапевтического действия AKG, стандартизированной очищенной фракции из масла печени глубоководной акулы (северных морей) «Экомер» (Ecomer®, Natumin Pharma АВ, Швеция) и неочищенного масла печени акулы (МПА).

С учетом результатов проведенного ранее анализа экспериментальных данных [3, 45], нами предпринята попытка обобщения сведений о клинических исследованиях продукта «Экомер» (Ecomer®), присутствующего на отечественном рынке, в части их противоопухолевого, иммуномодулирующего и гипохолестери-немического действия.
